What Is It?
This form is used to request insurance coverage for RF equipment or any equipment that must be insured under contractual requirements. It collects the details needed to add items to our insurance policy and ensures they are properly protected. Coverage is billed at a rate of $1.43 for every $100 of the equipment’s declared value.
