What Is It?
This is the form to be used to request to create/modify sponsored data within the oracle EBS.
How Do I Use It?
Simply fill in the required fields and then submit.
Entity Name:
When providing the Sponsor name, please provide the full entity name. This would include if the entity were a Corporation, LLC, Partnership, etc.
- Corporations: Inc./Incorporated, Ltd./Limited, Corp./Corporation
- LLC’s: LLC/Limited Liability Company
- Partnerships: LP/Limited Partnership, LLP/Limited Liability Partnership
If you’re unsure of the type of entity, search their website or ask them. Most businesses provide their legal entity name on their website. Additionally, most states have public sites where you can search business entity names. For example, NYS DOS: https://apps.dos.ny.gov/publicInquiry/
A lot of businesses also have DBA’s or Assumed Names that they go by. Please provide this name as well as this would be added in the site address. Example: QAC Company LLC, dba Monroe Quality Auto Repair
You’ll note, we do not abbreviate any name, street, address, etc. in the customer file
Please note:
Once the sponsor has been added to Oracle, they will be available in PACS the next business day.
Addresses & Contacts:
Each new entity created is automatically set up with a bill-to and ship-to contact with the entity’s address. If the billing or shipping address is different, please note and add to the Billing Contact section of the form. A billing contact person will no longer be listed in addresses for new requests. Instead, they will be added as an additional contact within the customer file.
Visual Compliance Report:

Tax ID numbers are now required. If you don’t have one at pre-award stage, please forward/provide at time of award. It will still be required at post-award.
Subs:
Please allow 1-2 additional days for processing time to appear in PACS. For Stony Brook and University at Buffalo, an email will be sent to the Campus Research IT Department to create the sub in your applicable system.
Sponsor Type Key:
- Federal - Government entity
- Foreign - Any entity outside US and it's territories
- Foundation - Nonprofit, has 'Foundation' in entity name
- Local Government - City, Town, Local Municipality
- Other - Other
- Private For-Profit - For-profit company (business and industry)
- State Government - State Entity (ex. NYS Department of Education)
